Process/Outcome Evaluator (Advocacy & Outreach) - Independent Contractor

Organizational Description: San Francisco Adult Day Services Network is a coalition of state-licensed, community-based adult day services programs offering a broad range of health, therapeutic and social services for frail elders and disabled adults. They also provide critically important support and respite for caregivers. The goal of adult day centers is to maintain or improve health and to prevent the need for institutionalization whenever possible. More information at www.sfadultday.org.

Project Description: The Network's Access to Care program has two major components, advocacy and outreach. The outreach component involves educating case managers, medical social workers, physicians, and others working in non-profit, healthcare, and governmental settings about the importance of ADS and how to refer patients/clients to ADS centers. The advocacy component involves identifying and addressing local and state-level system-related barriers to access to ADS for aging adults and people with disabilities. Position Summary: Under the supervision of the Executive Director and collaboration with the Access to Care Program Coordinator, the evaluator will be responsible for surveys to examine if the Network's Access to Care activities are reaching the intended audience and are occurring as planned (process), and results of progress toward meeting Logic Model objectives (outcome).

Duties and Responsibilities:
· Develop/implement/summarize evaluation surveys to measure knowledge of health care providers about adult day services and about potential post-advocacy efforts;
· Meet with the currently established Advocacy Evaluation Committee to monitor progress of, and advances in, policy reforms and needs of participants in Network centers;
· Develop/distribute/summarize evaluation surveys to Network centers and collaborating partners to review policy efforts and generate ideas for future advocacy. Qualifications:
· Experience in process/outcome evaluation for advocacy and outreach projects or related experience;
· Excellent oral and written communication skills;
· Strong initiative, creativity and self-motivation;
· Able to work independently with minimal supervision;
· Willingness and ability to travel within San Francisco to Novato/San Rafael, as needed, to capture data;
· Familiarity with local governmental and nonprofit agencies serving the elderly and disabled;
· Experience with logic models and organizing data for indicators;
· Familiarity with adult day services, other community-based health care modalities helpful;
· Enthusiasm for the mission and services of the San Francisco Adult Day Services Network;
· Strong, organizational skills. Ability to prioritize and manage multiple activities and deadlines;
· Demonstrated ability to operate successfully on an independent basis and as part of a cooperative team;
· Computer literate. The Network is PC-based and utilizes Microsoft Office products/software.

Working Conditions and Physical Demands: Work can be conducted by off-site telecommuting as well as in the Network's office in downtown San Francisco. There is travel to each of the centers in San Francisco and in Marin counties. The work emphasizes speech, hearing and vision.

Scope: This is a short-term, independent contract to be completed by July 2008. It is estimated that 40-50 hours are needed to complete the project. No benefits are provided. No relocation assistance available.

San Francisco

Develop and oversee the agency's adult day services (ADHC and ADCRC) programs; monitor the program's daily attendance to ensure optimal cost effectiveness; develop and implement annual goals, objectives and annual budget for the department; develop and implement operating policies and procedures; coordinate and supervise a multidisciplinary team to ensure timely assessments of and quality of care for participants.
